  • Be 18 years of age or older
  • You must be able to provide proof of your Missouri residency
  • You must have a diagnosis that qualifies in Missouri
  • You must submit a signed Physician Certification with your MMP application – and that is where we come in! We will provide the Physician Certification and then you can apply to get your card

  • A chronic medical condition that is normally treated with prescription medications that could lead to physical or psychological dependence, when a physician determines that medical use could be effective in treating that condition and would serve as a safer alternative to the prescription medication
  • In the professional judgment of a physician, any other chronic, debilitating or other medical condition, including, but n
    • Agitation of Alzheimer’s disease
    • Amyotrophic Lateral Sclerosis (Lou Gehrig’s Disease) 
    • Autism
    • Cachexia (also known as Wasting Syndrome)
    • Crohn’s disease
    • Hepatitis C

    • Inflammatory bowel disease
    • Neuropathies
    • Sickle cell anemia
